How to Install/Configure. Integration of Gravity Forms and Gravity Forms
– Registration page URL – this is the page the visitor sees when he/she registers for your website using your form. This can be any page you want on your site but it should have the “register” shortcode in it so you can access it later when configuring forms. If none of your pages have this shortcode, go to Appearance > Editor and paste the code into your theme’s header file (header.php.
– Contact page URL – this is the page the visitor sees when he/she fills out your contact form and clicks “send”. If none of your pages have this shortcode, go to Appearance > Editor and paste the code into your theme’s header file (header.php. Then create a new page on your site with this same code and name it “Contact Form” or something similar.

The fields you add can be anything you want them to be but they need to fplow certain formatting requirements, as explained here. https://www.gravityhelp.com/documentation/gravity-forms-documentation/fields/form-field-data-types/.
